- Case management and carrying out needs assessments in tandem with a qualified nursing specialist

Main responsibilities
- Care, support and advice for our clients at home Ensuring the care process
- Case management and carrying out needs assessments in tandem with a qualified nursing specialist
- Creation, adaptation and evaluation of care documentation Conducting consultations Correct recording of performance data
- Collaboration with relatives, doctors, hospitals, homes and other specialist departments
- Integrated 6-month external internship in inpatient acute and Transitional care
- Duration of training: 3 years (shortened HF course of 2 years possible if you complete your apprenticeship as a health specialist EFZ) or 3 to 4 years in part-time training
- Own learning transfer training (LTT), incl.
- Regular exchange with the HF students from the partner companies Requirements:
- Completed apprenticeship as a health specialist EFZ, Matura or equivalent
- Good knowledge of spoken and written German (at least level C1) Modern IT knowledge (MS Office, Perigon)
