The post shall be limited in time from 1 December 2026 to 31 December 2027, with an option of renewal. Tasks: • You advise patients with heart failure in the heart failure counselling team as part of your extended nursing skills and actively contribute to the further development of this offer within the LUKS Group • They look after patients with telemedical care shares together in the interprofessional team of integrated care with cardiologists, family doctors and specialists of Spitex • You take on a central coordination role in this new care system with telemedical components and actively shape its development and implementation as part of a study • They are actively involved in the further development of the care of patients with heart failure within the LUKS Group and beyond. Requirements • You have a completed master’s degree in care as an APN, preferably towards NP and are interested in heart failure • You assess complex questions in a structured manner, set priorities in line with the situation and independently derive suitable measures from them. • They advise employees appropriately and are also professional and binding in medical or organizational issues • In interprofessional cooperation, you actively contribute your professional perspective and shape new structures and processes in a solution-oriented manner.
