- In an interdisciplinary team, with case managers, FaGes and housekeeping and support staff at your side, it should be a new form of work that is contemporary and allows for the necessary flexibility, but also structure and continuity. You have the following qualifications:
- Completed training for the dipl. Care professional HF/FH Ideally Spitex experience Strong professional and social skills
- self-employed and responsible working practices
- valid category B ticket Your tasks: As a case-leading employee, you control the care process from the first visit with the clarification of needs after interRAI until completion.
- You are the interface for interprofessional cooperation with all involved entities, advise relatives and accompanying health professionals as well as the care team.
- You support our customers holistically and resource-oriented. We offer you:

- 5 weeks of holidays (from 50 years of age 6 weeks);
