- Provide access and daily medical follow-up to patients in the service (present 60-80% spread over 5 days).
- Write reports and exit letters in close collaboration with the health care team.
- To be a key player in medical coordination, in connection with the specialists of the health home and the technical plateau.
- Participate in a friendly hospital service, oriented to the quality of the routes and the relevance of the care.
- Working in an innovative interprofessional context, with great freedom of clinical and organizational initiative.
- Adjust the trajectories of patients, often complex, to ensure appropriate and progressive management.
- Actively contribute to institutional development, including the evolution of the UATM concept as a credible alternative to heavy hospitalizations. Post conditions
- Activity rate: 60-80%, regular presence over five days.
- No penalty, thanks to the permanent presence of an internal emergency service.
