Application process
- The CHAD brings together the teams from the application line (LAD), IMAD link to the HUGs, IMAD arcade to the HUGs, the Mental Health and Psychiatry Team (ESMP), Multi-purpose Home Hospitalization (HAD-poly), Interprofessional Oncology Team (EISO), Paediatric Care and Family Respite, Home Palliative Care Consultation (CospaDom), the Geriatric Coordination Team (COGERIA), the Night EMD, as well as POOL CHAD and the long-term centralized pool.
- Connected to the team leader (RO) and integrated with the EMP, the nurse coordinator IC organises the support and care activities of an EISO team, ensures harmonised management of customer flows and acts as a contact person for case reference, on aspects of quality and adequacy of services.
- It promotes customer orientation in all actions undertaken through a multidisciplinary and interprofessional approach.
- Process new applications at the clinical level, assign references under the control of the ROE, and plan care and assessments in advance, taking into account available resources

Company and context

- Ensure relationship and coordination with the client care network
- Conduct client tour planning, keeping informed and integrating all new information received, in collaboration with other EMP members
- Contributing, with the team's administrative assistant, to the smooth conduct of the invoicing of benefits and its compliance with the requirements in force
- Promote the development of interdisciplinary practice and objective work for all services provided, ensuring the delegation of benefits to the right professional, and providing community clinic support to referrals
- Actively participate in the upward and downward transmission of information, promoting a positive climate of exchange within the team, as well as with partners.
- HES-SO Bachelor of Nursing or equivalent training
- Successful experience of at least two years in coordinating care
- Successful experience in oncological care is a plus
- Ability to work independently and interprofessionally in a multidisciplinary team
