The specific field of the liaison nurse covers imad clients (known or future clients) hospitalized, requiring a return home with imad services. Its scope of activity includes needs assessment, medical validation of required services, forward-looking coordination of interventions by home support teams (EMD) and/or centralized services. Your missions and responsibilities

• Inform customers or future customers of the home services offered by imad, partners of the Geneva socio-health network, prices and reimbursement terms for home support services. If necessary, direct the client to the person or structure who can advise them.
• Ensure coordination between imad and the private/public hospital sector to organize the return home of clients, with services provided by imad.
• Promote the exchange of useful and necessary information for customer care (according to the planned documentation).
• Facilitate hospital discharges for clients and encourage a return home in the best conditions.
• Collaborate with the client, the caregiver, health and social professionals.
• Prevent inappropriate (re)hospitalizations.
• Work in close collaboration with the imad arcade team located at HUG.
• HES-SO Bachelor in nursing or training deemed equivalent and registered by the Swiss Red Cross
• Knowledge of the care process and clinical pathways, of the Geneva social health network
• Ability to work alone and interprofessionally within a multidisciplinary team
• Sense of responsibility and solution orientation, sense of communication, negotiation and relationships with others
• Ability to adapt and delegate
• Stress and conflict management
• Resistance to physical work.
