- As ICLS, you will play a key role in transforming home care by contributing to continuous improvement in the quality and safety of care.
- Your mission will be part of a collaborative, patient-centred approach as a full partner. Your main responsibilities:
- Support the institutional mission to maintain home in a supportive, safe and patient-friendly environment.
- Initiate, coordinate and support clinical interventions related to the function of ICLS with care teams and as appropriate to patient needs.
- Assess complex care situations and respond with evidence.
- Participate in interdisciplinary clinical decisions in the transition, in collaboration with care settings and partners, to develop individualized care pathways integrating clinical assessment, therapeutic education and professional support etc.
- Actively contribute to the implementation phase of advanced nursing practice, by being a proposal force or any clinical implementation project.
- Support the quality of clinical documentation. Specific areas of intervention:
- Management of clinical projects , particularly related to the implementation of new professional practices.
