Full description

• Attached or attached to the Chief of Palliative Care Services, you supervise multidisciplinary care in accordance with procedures established by the Centre.
• You coordinate the network of palliative care referrals in collaboration with the departments as well as the various activities of the centre (organisation of symposium, RESOPAL)
• You are working with the Patient Manager (IPM) and RES to improve support. You are involved in the coordination of the various projects under way. You train the collaborators involved in identifying patients eligible for standardized or complex care. As well as the use of standardized process and management assessment and monitoring tools. You implement and follow the relevant indicators (e.g. coverage rates for support). You improve the quality of care by offering quality actions and participate in the promotion of the Centre within the network.
