As an assistant or social worker at the HUGs, you intervene with the patient of the Geriatric and Rehabilitation Department in a comprehensive and multidisciplinary approach. You ensure the social follow-up of hospitalized patients, especially in rehabilitation units and waiting for placement. Your role is to assess the social needs of patients and their families, to identify social difficulties and to propose appropriate solutions. You intervene in the management of administrative and financial demands, and accompany patients in their home maintenance, rehabilitation or transition to another institution (EMS, care facilities). You work closely with the internal multidisciplinary team and external partners. Your work is based on individual, family and multidisciplinary talks to build trust with patients, understand their situation and develop concrete and personalized solutions. You are also responsible or responsible for the rigorous transmission of social needs and patient projects to the healthcare team and external partners. Finally, you are actively involved in professional monitoring of social and legislative issues, in particular social insurance, and in the prevention of social problems. You are involved or involved in the continuous improvement of practices within the service.
