The ARCADE liaison nurse (IDL) is responsible for informing, directing and recording requests from the public and in particular from patients, future patients and caregivers in order to ensure that the quality of care is maintained. The IMAD arcade, located in the Gustave Julliard Building of the HUG, is attached to the liaison service and is present 7 days a week. The IDL ARCADE works in close collaboration with the liaison nurses of the various liaison sites and ensures the relay of their activity on weekends and public holidays. The specific scope of the liaison nurse covers hospitalized patients requiring a return home with nursing care services. Its scope of activity includes needs assessment, medical validation of required services, and forward-looking coordination of interventions by home care providers. Your missions and responsibilities

• Inform the public and HUG staff of the home services offered by IMAD, partners of the Geneva socio-health network, the prices and reimbursement terms for home support services. If necessary, direct the patient to the person or structure who can advise them
• Ensure telephone response from the Proch’infos line
• Ensure coordination between IMAD and the hospital sector to ensure the monitoring of hospitalized IMAD patients
• Ensure coordination between the home care provider and the hospital sector to organize the return home of patients requiring nursing care
• Ensure administrative registration of new IMAD patients
• Promote the exchange of useful and necessary information for patient care (according to the planned documentation)
• Facilitate hospital discharges for patients and encourage a return home in the best conditions
• Contribute to the promotion and prevention of health by mastering the issues and the link with the Geneva socio-health network
• Collaborate with the patient, the caregiver, health and social professionals
• Prevent inappropriate (re)hospitalizations
• Work in close collaboration with the IMAD arcade team located at HUG.
• Bachelor HES-SO in nursing or training deemed equivalent and registered by the Swiss Red Cross, authorization to practice in the canton of Geneva
• Knowledge of the care process and clinical pathways, of the Geneva social health network
• Ability to work alone and interprofessionally within a multidisciplinary team
• Sense of responsibility and solution orientation, sense of communication, negotiation and relationships with others
• Ability to adapt and delegate
• Stress and conflict management.
