- Your missions and responsibilities
- Inform clients or future clients of the residential services offered by imad, the partners of the Geneva social health network, the rates and the terms of reimbursement of home care benefits.
Main responsibilities

- Inform clients or future clients of the residential services offered by imad, the partners of the Geneva social health network, the rates and the terms of reimbursement of home care benefits.
- Depending on the need, refer the client to the person or structure who can advise the client.
- Ensure coordination between imad and the private/public hospital sector to organize the return of clients, with services provided by imad.
- Promote the exchange of useful and necessary information for the care of the client (according to the documentation provided).
- Facilitate the discharge of clients from hospital and promote a return home in the best conditions.
- Collaborate with clients, caregivers, health and social professionals.
- Prevent inappropriate (re)hospitalization.
- Work closely with the Imad arcade team at the HUG.
- Bachelor HES-SO in nursing or training deemed equivalent and registered by the Swiss Red Cross
