Main responsibilities
- Care, support and advice of our clients at home Ensuring the care process
- Case management and implementation of needs assessments in tandem with a dipl. Nursing professional
- Preparation, adaptation and evaluation of care documentation Consultation services Correct collection of performance data
- Cooperation with relatives, doctors, hospitals, homes and other specialist centres
- Integrated 6-month internship in inpatient acute care and transitional care
- Duration of training: 3 years (shortened HF studies of 2 years possible at apprenticeship qualification as a health EFZ) or 3 to 4 years in part-time training
- Own learning transfer training (LTT), including regular exchange with the HF students from the cooperation companies Requirements
- Completed apprenticeship as a specialist Health EFZ, Matura or equivalent
- Good knowledge of German in word and writing (at least level C1)
- Contemporary IT skills (MS Office, Perigon)
