• Resource and goal-oriented care: You take care of adults of all ages, focus on their individual resources and set clear care goals.
• Case Management & Care Process: You are responsible for the evaluation, planning and control of the entire care process and ensure that it runs smoothly.
• Counselling and accompaniment: You conduct independent conversations, accompany therapeutic processes and support affected persons and their relatives competently in dealing with everyday stresses.
• Care documentation: You keep the electronic care documentation seamless, transparent and traceable.
• Instruction and coaching: You support and advise team members of all skill levels, pass on your expertise and promote team development.
• Flexible workplace: You can do your office work at our location in Bern, while your area of application extends directly to customers throughout the city of Bern
