Tasks
You work in transitional care and care for our customers after acute illness, accident or chronic illness, with the aim of accompanying them home or into a new suitable way of living and living. You actively take responsibility for the design and implementation of the care process and also contribute to a smooth daily routine of the residents and their relatives. Furthermore, you act as an important contact person and ensure the management of electronic care documentation and the application of the RAI system in relation to care standards.
Requirements
- Secondary education (FaGe, DNI, FASRK, nurses, home care workers, etc.)
- Mind. 2 years of professional experience in acute care
