Role overview

They advise our patients independently and accompany them and their reference system in difficult life situations in connection with illness and hospital stay.
They arrange and coordinate external offers, advise patients and their relatives on inpatient connection solutions (age and nursing home, rehabilitation and recreation) as well as outpatient care and support offers and organize them.

- They advise our patients independently and accompany them and their reference system in difficult life situations in connection with illness and hospital stay.
- They arrange and coordinate external offers, advise patients and their relatives on inpatient connection solutions (age and nursing home, rehabilitation and recreation) as well as outpatient care and support offers and organize them.
- They perform administrative tasks, keep patient records, history records and statistics.
- They actively participate in the interdisciplinary exchange at the acute care rehabilitation station.
- They organize and lead interdisciplinary family meetings.
- They are well-versed in social security issues related to illness and accident, and provide financial resources.
- They coordinate the networking of patients with the Pro Senectute, the social services, the KESB and other third parties. Requirements:
- You have completed a bachelor’s degree in social work FH BSc/B.A., a degree in social education HF or an equivalent education and/or bring a CAS in case management and ideally experience in clinical social work.
- They encounter people in challenging health situations with empathy and appreciation and show skill in talking to predominantly older patients.
