Case Management Hirslanden Bern operates at three locations: Salem Spital, Klinik Beau-Site and for Klinik Permanence. The goal is to organize a suitable, timely inpatient follow-up solution for our patients, as part of a rehabilitation, spa stay or home entry. We focus on a close exchange between doctors, nurses and relatives. For us, the optimal care of the patient with simultaneous cost control of hospital stays is in the foreground. YOUR TASKS

• You organize timely, suitable inpatient follow-up or transition solutions for our patients
• You advise and accompany patients and their relatives during the entire exit management process
• Together with internal and external bodies, you coordinate the implementation of the transfer of patients to other institutions
• You work constructively with the interdisciplinary teams
• You are actively working to continuously optimize cooperation with our external agencies (social insurance, various clinics, homes, KESB, etc.)
• With the support of the DRG team, you have a data basis to make our exit management as efficient as possible.

Your profile

• You have completed basic medical or nursing training as well as professional experience in case management – ideally supplemented with further training in case management
• You are characterized by a sense of responsibility and an independent, flexible and loyal personality with a strong organizational talent
• Through your solution-oriented as well as networked way of thinking and acting, you understand connections and can act accordingly
• Thanks to your strong social skills and communication skills, dealing with different stakeholders is easy.
• We require stylish knowledge of German and every other language is an advantage
• You are adept at dealing with Microsoft Office
