- accompany and support people with psychiatric diseases in their home environment
- Diploma as a nurse HF (preferably with experience in psychiatry) Joy of ambulatory psychiatric care
Main responsibilities

- accompany and support people with psychiatric diseases in their home environment
- assumes responsibility for the entire care process including case management
- Work closely with internal and external specialists
- bring your expertise and experience to the individual support of our customers Document and plan with interRAI CMH
- work independently, responsibly and flexibly You bring that with you
- Empathy, resilience and a solution-oriented working method
- Category B driving licence You can look forward to that.
- Regular working hours from Monday to Friday a small, motivated and humorous team A lot of personal responsibility and scope for design
- Independent planning of your customer and office appointments
