Care specialist HF with drop guidance (50-100%)

• What awaits you:
• Self-employed, competent and resource-oriented basic and treatment care for clients of all ages with a wide range of illnesses
• Takeover of case management for a defined client group
• Responsible for planning, controlling and monitoring the care process
• Conducting assessments and location discussions with RAI-HC
• Supporting our clients in a self-determined everyday life at home In the case of a ensemble of about 50%, there is the possibility to apply in the morning in order to ensure the compatibility of family and profession
• At a 100% Pensum you work on 1-2 weekends a month
• What you bring:
• Completed training as a qualified care professional HF
• professional experience and broad expertise
• An independent, responsible way of working
• open-mindedness, flexibility and empathy
• willingness to travel by e-bike or car
