Nurse HF Department of Dementia (60-90%)

Holidays

Independent, competent and resource-oriented care of our clients and their relatives in dealing with dementia in their usual living environment
Assuming case management for a defined group of clients
Planning, controlling and monitoring the care process
Supporting our clients in a self-determined everyday life at home by promoting independence and quality of life
Interdisciplinary cooperation with relatives, doctors and other services
Conducting assessments and site discussions with RAI-HC
What you bring:
Completed training as a qualified nurse HF
Professional experience and broad expertise
Additional training in the field of dementia beneficial (or willingness to continue training in this field)
An independent, responsible way of working
High empathy and empathy
Willingness to travel by car
