• You take over the management of the care and care process of your assigned clients:
• You are responsible for the case management and carry out the demand survey with the interRAI HC
• You take responsibility for the implementation of the nursing diagnostic process
• You are an important professional contact for our employees and support them with professional questions
• You support in complex care situations and wound treatments
• Together with the team, you ensure a high and continuous quality of care

• Diploma as nurse / nurse specialist HF, DN II, AKP or a tertiary education recognised by the SRC
• User knowledge in Perigon and interRAI HC is an advantage
• High level of social and professional competence
• Self-employed, reliable and responsible work

• Very good oral and written knowledge of German
• Category B driving licence
