LAMal: the Swiss compulsory health insurance

LAMal (Federal Health Insurance Act, KVG in German) is the backbone of the Swiss health system since 1996. Everyone domiciled or working in Switzerland must be insured with an approved health fund. For cross-border workers resident in Italy and working in Canton Ticino, LAMal is one of two options — the other being the Italian National Health Service — and the choice (called right of option) must be exercised within three months of starting work and is irrevocable.

2026 premiums for Ticino cross-border workers

LAMal premiums for cross-border workers in 2026 range from around CHF 280 to CHF 650 per month per adult, depending on the fund, deductible (CHF 300 to 2,500) and insurance model (standard, family doctor, HMO, telemedicine). The 14 funds authorised for Ticino cross-border workers are Helsana, Swica, CSS, Sanitas, KPT, Visana, Sympany, Atupri, ÖKK, Concordia, Sodalis, EGK, SLKK and Rhenusana.

LAMal vs SSN: the strategic decision

There is no one-size-fits-all answer: the choice depends on health status, family composition and preferences. LAMal is typically better for families with young children or chronic conditions; SSN is better for healthy young workers living near the border with good Italian hospitals. On a CHF 5,000/month salary, LAMal costs 7-11% of income; SSN is free but with longer waiting lists.

For new cross-border workers (from 17 July 2023) LAMal premiums are tax-deductible in Italy up to EUR 3,000/year under Article 3 of the New Agreement. Fund switches within LAMal are allowed annually by giving notice by 30 November for effect on 1 January, or by 31 March / 30 September for optional deductible changes.
