These include the failure to draw up the Risk Assessment Document, the Operational Safety Plan and the Plan for the Assembly, Use and Disassembly of scaffolding, the irregular application of the Safety and Coordination Plan (PSC), the use of non-conforming scaffolding, ladders and equipment, the failure to prepare provisional works and suitable protections against the risk of falling from a height, as well as further organisational and documentary deficiencies.

According to the findings of the investigations, the 18 suspended companies committed serious and repeated violations. For example, one of the companies, active in the construction of a building in Bellinzona, did not draw up the Risk Assessment Document, as required by current legislation (Law 81/2008). This resulted in a high risk for workers, who were exposed to risks of falling from a height and injury caused by non-compliant equipment.
